1) Complete the Online eConsent <—click link
2) CALL or TEXT (303) 724-1275 or EMAIL Questions@ASKhealth.org to make a "LAB ORDER request"
3) You will receive a confirmation message from ASK when your LAB ORDER is complete — typically 3 business days after request was made
4) Go to your preferred Outpatient Lab (below) during normal business hours (as listed) for your ASK screening. No appointment is required.
** NOTE: Only children can get screened at Children's Hospital locations **

US families interested in the ASK screening who live further than 30-miles from any screening site (above), can receive an AT-HOME SCREENING KIT (finger poke) to your home.
